Who this notice applies to

This notice applies to all clients of Allison Puryear, LCSW, whether services are provided in person in Asheville, NC or via telehealth across North Carolina and Washington State.

Minors

I do not provide therapy services to minors through this practice.

As a client, you have the right to:

  • Request restrictions on how your information is used or disclosed
  • Receive a copy of this notice
  • File a complaint if you believe your privacy rights have been violated

Filing a complaint

If you believe your privacy rights have been violated, you may file a complaint with my practice or with the U.S. Department of Health and Human Services Office for Civil Rights. Filing a complaint will not affect your care.

Data Security

Client records are stored securely through HIPAA-compliant practice management software. Consultation requests submitted through the client portal are encrypted in transit and at rest. Session notes and clinical records are retained in accordance with North Carolina and Washington State professional licensing requirements.

This website is hosted on Netlify, which uses HTTPS encryption for all traffic. No clinical or personal health information is stored on this website.
